Disease Management
Disease Management services are available to beneficiaries at Peterson and Schriever Space Force Base.
Services Provided
We provide education and support to beneficiaries diagnosed with chronic diseases such as:
- Hyperlipidemia
- Diabetes and Pre-Diabetes
- Asthma
The goal of our office is to help minimize complications in patients with a particular chronic disease and help lessen the long-term usage of health care resources for that specific population.
Additionally, they provide important health education and advocacy by helping patients follow their treatment plans and gain control over and become more proactive in their healthcare.
Referrals
Services are available upon referral from their primary care manager for individualized education or in-classroom format. Patients who have previously been diagnosed with one of the chronic diseases mentioned above may self-refer.
