Patient-Centered Medical Home
Recent implementation of the Patient‐Centered Medical Home model of care provides greater satisfaction to patients, physicians, and the health care team by ensuring that patients' needs are met and access to care is improved, health information is handled effectively, and the team works as a cohesive unit.
How to Make an Appointment
Acute and established medical appointments are offered during normal duty hours from Monday to Friday, except the second Thursday of the month when we are closed for the entire day for Medical Readiness Training, and can be made by calling the appointment line at 719-524‐2273.
Walk-in services are available for suture removals, blood pressure checks, wart treatments, and Depo‐Provera injections when the COVID-19 Pandemic is under control.
Family Health Clinic walk‐in times are from 8:00 a.m. to 9:30 a.m. and 1:00 p.m. to 2:00 p.m. Patients empaneled to the clinic may also leave a message with their Primary Care team by calling the appointment line at 719-524‐2273 or utilize our secure messaging system.
Patients can come by the Family Health Clinic to register into the system and begin messaging your healthcare provider team for such things as medication refills and referral renewals.
